All active members of the Philippine Health Insurance Corporation (PhilHealth) may now access their membership records and Philhealth contributions in the internet via the Member Inquiry facility.
The facility allows PhilHealth members to view their PhilHealth records in the convenience of their home or office without the need to go to a PhilHealth office. It also allows the members to view Member Data Record (MDR) to see if list of dependents is intact and to view if premium contributions are reflected.
Your online record doesn’t cover checking if you are qualified to avail of PhilHealth benefits.
Before you start
You’ll need your PhilHealth identification number, email address and phone number. If you don’t know your PhilHealth ID number, you can ask your employer’s human resources department (HRD) to get it.
Step 1. Go to PhilHealth website (www.philhealth.gov.ph) then on the right sidebar, you will see a member inquiry form. Since you are not yet register, click the “Register” link just below the login button or left side of “Forgot Password” link.
Step 2. Supply the required (*) data using the PhilHealth registration form. After providing your PhilHealth number (without the dash -) and other personal information and completing three security questions and answers and the security code, click “Submit Registration.”
Step 3. A popup confirmation window will appear after you submit your registration confirming your registration has successfully sent and you need to check your email for activation link.
Step 4. Check your email account that you used when doing Step 2 for PHILHEALTH Member Information System activation link. The email also contains your PIN and Password that you will use when you login online.
After clicking the activation link, you will then be redirected to a page confirming that your account is activated, click the Login link to be redirected to the homepage.
Step 5. You may now use the Member Inquiry facility using your PIN and password. Just provide the answer to the security question to successfully login to member’s area.
Inside the member’s area are links to “Member Profile,” “Reset Password,” “Premium Contributions,” and “MDR Printing.”
