November 2014 NLE passers may find here information regarding registration, oathtaking schedule, and verification of ratings for November 2014 Nurse Licensure Examination (NLE).
Successful examinees, who took the series of examinations in Manila, Baguio City, Laoag, Angeles, Cabanatuan, Cebu, Dagupan, Davao, Iloilo, Legazpi, Lucena, Pagadian, Tacloban, Tuguegarao, Zamboanga, Cagayan de Oro (CDO) and Bacolod, are required to register, and take oath such that they can exercise nursing profession legally.
Pursuant to Section 16, of Republic Act No. 9173, “all successful candidates in the examination shall be required to take an oath of professional before the Board or any government official authorized to administer oaths prior to entering upon the nursing practice.”

November 2014 NLE passers registration schedule and requirements
Registration for the issuance of Professional Identification Card (ID) and Certificate of Registration will start on February 9, 2015.
Those who will register are required to bring the following:
- duly accomplished Oath Form or Panunumpa ng Propesyonal,
- current Community Tax Certificate (cedula),
- 1 piece passport size picture (colored with white background and complete name tag),
- 2 sets of metered documentary stamps,
- 1 short brown envelope with name and profession,
- Initial Registration Fee of P600
- Annual Registration Fee of P450 for 2015-2018
Successful examinees should personally register and sign in the Roster of Registered Professionals.
MANILA AND PAMPANGA PASSERS
The Professional Regulation Commission (PRC) hereby informs all those who passed the November 2014 Nurse Licensure Exam in Manila and Pampanga of the schedule of their initial registration at the 3rd floor, PRC Annex Building in Manila:
Registrants are advised to strictly observe the schedule for initial registration to avoid any inconvenience.
BAGUIO PASSERS
Registration starts on February 2, 2015 (Monday). To decongest the Regional Office and to enable passers to save on travel time and expenses, the Regional Office will process initial registration outside of Baguio City. Mobile outreach dates and venues are:
The Office will process initial registration on first come, first served basis; no alphabetical queuing. Those who will register are required to bring the following:
Duly accomplished Oath Form (downloadable at www.prc.gov.ph Online Registration System);
2015 Community Tax Certificate (Cedula);
One (1) piece passport-size picture with white background and complete nametag (Surname, first name, middle name) ; and
One (1) documentary stamp; and
Payment of Initial Registration Fee of P1,050.00
Professional ID Cards (PICs) will be issued after the Oathtaking ceremonies tentatively scheduled on the following dates:
February 21, 2015 – Batac City, Teatro Ilocandia; 2:00 PM
February 27, 2015 – Cabanatuan City, Wesleyan University Philippines
February 28, 2015 – Dagupan City, Lyceum Northwestern University Gymnasium; 9:00 AM
March 4, 2015 – Baguio City, Prince Bernhard Hall, Saint Louis University; 9:00 AM
LUCENA PASSERS
Notice is hereby given that the registration for the nurses who passed the NOVEMBER 2014 Nurse Licensure Examination (NLE) will run from FEBRUARY 9-16, 2015. Note that the schedule below is only for those who took the exam in Lucena City.
November 2014 NLE Takers verification of ratings (VoR)
Through Verification of Ratings (VoR) online on PRC website, November 2014 NLE takers will be able to check grades by entering the required information in the appropriate fields.
